Student Enrollment Process

Step 1: Complete the Student Interest Form

Visit www.revivedmta.com and complete the Student Interest Form located on our homepage. This is the first step in beginning your enrollment journey. Once submitted, the form will guide you through the next steps of the enrollment process, including scheduling your consultation and receiving additional enrollment instructions.

Step 2: Schedule Your Free Enrollment Consultation

Prospective students will be guided to schedule a free consultation with our Enrollment Specialist to discuss program options, tuition, payment arrangements, schedules, and admission requirements.

Consultation Hours:

Monday–Thursday: 8:00 AM–4:00 PM
Friday: 8:00 AM–12:00 PM

Step 3: Select Your Program

Choose the healthcare training program that best aligns with your career goals, schedule, and financial needs. Our Enrollment Specialist will review program expectations, certification requirements, and available tuition assistance opportunities.

Step 4: Complete Enrollment Requirements

Submit your enrollment application and all required supporting documentation. Students must meet the admission requirements for their selected program.

Step 5: Review Tuition and Finalize Enrollment

Review tuition, enrollment deposits, payment plans, and applicable funding opportunities. Students must complete their Enrollment Agreement and satisfy all initial financial obligations to finalize enrollment.

Step 6: Attend Mandatory Student Orientation

All enrolled students are required to attend orientation before beginning classes. Orientation covers the Student Handbook, academic expectations, attendance policies, professionalism, dress code, financial responsibilities, and certification requirements.

Step 7: Attend the First Day of Class

All students must attend and actively participate on the first scheduled day of class, whether enrolled in an in-person, hybrid, or online program. Failure to attend without prior written approval from administration may result in forfeiture of the student's seat.

Important Enrollment Reminders

Enrollment closes seven (7) days before the scheduled class start date.
Completing the Student Interest Form does not guarantee admission or reserve a seat.
All enrollment documentation and financial requirements must be completed before classes begin.
Orientation and first-day attendance are mandatory.
Class placement is subject to availability and completion of all admission requirements.

Academic Requirements

Your Gateway to Learning

1. Gather required documents to apply which includes:

a. Proof of successful completion of public, private, or home schooling at the high school level or obtainment of a recognized high school equivalency credential;

b. Evidence that you are able to read, write, and speak English; (high school diploma, GED, Interview)

c. Immunization Records from your local health department or by attempting this link:

d. Be in good health and able to bend, twist, lift at least 50 pounds, and be free from communicable disease;

d. Not be listed on the Nurse Aide Registry or Misconduct Registry (for the State of South Carolina) in revoked status; (School will verify this status)

*When a student applies to the school they are giving permission for the school to conduct a criminal history check, verification check with the Nurse Aide Registry, and the Misconduct Registry in the State of South Carolina.

e. Complete TB test and drug screen with negative result.

2. Complete application to the school.

3. Pay for course when registering at the physical location of the school 2724 W. Palmetto Street Florence, SC 29501.

6. Submit all documents together at one time (items 3-5).

Costs and Subsidies

The Information You Need

Total Program Cost: Tuition varies by program. Please refer to the tuition amount listed under your selected program or contact the Academy for current pricing.

Tuition & Included Services: Program tuition includes instructional services, required background screening, and PPD (tuberculosis) testing, as applicable to the selected program.

Textbooks & Instructional Materials: Required textbooks, course materials, instructional supplies, handouts, and access to designated learning platforms are included in tuition unless otherwise specified. Computers and electronic devices are not provided for students to keep unless explicitly stated in the Enrollment Agreement.

CPR Certification: Eligible students receive American Heart Association (AHA) Basic Life Support (BLS) Healthcare Provider certification, valid for two years, in accordance with program requirements and Academy policies.

National Certification Examination Fees: Revived Medical Training Academy covers one initial NHA certification examination attempt for eligible students who successfully complete their program requirements and take the examination within 60 days of program completion. Additional attempts and examinations taken after the 60-day deadline are the student's financial responsibility.

Nursing Assistant (CNA) Certification: Credentia examination fees are covered for one initial attempt only when included in the student's tuition or approved funding agreement. Otherwise, students are responsible for paying the applicable examination fees directly to Credentia. All additional attempts are the student's responsibility.

Sterile Processing (HSPA) Certification: HSPA certification examination fees are the student's responsibility unless otherwise specified in the Enrollment Agreement.

Accepted Payment Methods: Payments may be made by cash, credit or debit card, or money order, subject to the Academy's accepted payment procedures.

Flexible Payment Options: Payment plans are available for eligible programs. Students are encouraged to discuss payment arrangements, deposit requirements, and available tuition assistance opportunities with the Enrollment Specialist before enrolling.
